This policy explains how MYObase collects and uses information when you visit myobased.com, complete a form, take the Healthspan Snapshot, or request information about our services.

Information we collect

We may collect your name, email address, form responses, referral source, and technical information such as browser type, pages visited, and campaign parameters. The Healthspan Snapshot is an educational self-assessment for general wellness. Please do not submit medical records, diagnoses, or other sensitive health information through the site.

How we use information

We use information to provide the Snapshot, respond to requests, send relevant follow-up communications, improve the site, understand campaign performance, and operate MYObase services. We do not sell your personal information.

Service providers

Form and Snapshot contact information is sent to GoHighLevel (GHL), our customer relationship management provider, so we can store the contact record and send requested follow-up. We use Plausible Analytics, when configured, to understand aggregate site use. Plausible is designed to operate without cookies and without collecting personal data for cross-site tracking. We may also use service providers that help us host and secure the site.

Your choices

You may opt out of marketing emails using the unsubscribe link in an email. You may also ask us to update or delete your contact information, subject to legal and operational recordkeeping needs.
